In 2021, LHF assisted the Cook County Department of Public Health in completing its IPLAN, called WePlan 2025, which had been put on hold during the COVID-19 pandemic.
LHF supported the Village of Schaumburg in a two-phase process from 2023 to 2025 to complete a health services needs assessment and community health improvement plan.
